Does anybody have a sample of the message sent by SRM when making a punchout request that they would like to share?

I still can't find an example of the actual call to the supplier's site. Does anyone have an example that is laid out specifically:

http://<supplier site>.com/catalog.php?USERNAME=xxxxxx&PASSWORD=xxxxx&HOOK_URL=http://<your sap server>.com
