Upload problem with the form

Dear All,

We have designed a PDF interactive form for appraisals and we are able to send the same from R3 to the outlook mails and can receive the filled forms also.

We are unable to upload the values mentioned in the form to the R3 data base. Can you please suggest a way as to how to upload the values.
