Solution Manager and HP-Quality Center

I am trying to integrate Solution Manager and HP-Quality Center. Can you someone tell me what the process is. What is the process to install the adapter.

Also what SAP Notes need to be implemented.

We are on Solution Manager SP 18 and HP-QC 9.2
