Guided Procedure - Cannot retrieve template structure

Getting Cannot retrieve template structure error when I try to create form using imporsonalized Interactive form ?

Guided Procedures --> administration --> Interactive forms --> manage Impersonlized forms --> select your form --> create form and select data and then getting the following error

Cannot retrieve template structure: Execution ended in an unrecognized state

Anyone has come across this issue. We have deployed credentials and Fp_TEST_00 is working fine.

Any help ?

Looks like ADSUSER does not enough rights. I assigned administrator group access and it worked fine.
