X Symbol in Work Flow Applet

Hello All

Does any one saw a scenario like this -

1 - When you open a SC and open Approval

2 - Instead of opening Java Applet , it show you a X Symbol and tell Application error in table view.

If yes , how to track or monitor that how many SC have a scenario like this ? And also how to fix this ?
