OSS are online sap support notes. These notes are available online for solving critical problems in sap system.We may use the already existing notes or may add our own quaries.
In order to apply any OSS note, you have to be authorized by your company and be assigned an OSS ID and password.
For any SAP standard program modification, you are required to login to OSS and request for a repair program Access key
OSS is an online SAP service portal which will have all kinds of updates about SAP patches and latest note
information. I feel no one will ask what it contains? if some ask like this you can ask him raise a SAP message to get the answer.
So you need to follow the instructions, written in these notes and you will have your problem solved. The numbers I have written are the service packs of SAP, provided to solve the issue of reversing the UD.

Hope you are talking abt 03 inspection. Try to confirm the QM operation through co11n. If you have set error in the Operation confirmation configuration ( for compulsory result recording) change it and do the operation confirmation through co11n
You can only do results recording after UD if you have set your MIC characteristics for the operation to be long term characteristics. If this setting is not in place then you can not record results after usage decision. If you only want to change the usage decision you can do this in transaction QA12.
