Hi All,
We are changing the IP-address for the sap server, how can we find all the udp and tcp ports required for communication of server to other servers.
Apart from the port number listed in the instance profle and the service file at O.S level.
Any others caution to be taken and any other information that i missed

You can find those ports defined on the services file located on
UNIX
/etc/services
WINDOWS
<drive>:\windows\system32\drives\etc\services

You did indeed completely miss to give any information on your system! How can anyone help you, when we don't know what you need.
