Is there anyway to create checkboxes in smartforms? Even if we do create them, would it appear in the print preview?

Click the Editor button from the Text element's 'General attribute' tab, an editor window will open. From the menu bar navigate Incude->Characters-> SAP symbol , choose the filled circle.
You can't view it in the print preview, but you will get it in the print.

you can use the symbols in the text editor of the smartform, in which ever window you want.
